Know about the most common causes of infertility

Although one out of every six couples has problems fertility but in the end most of them have children with or without treatment. The most obvious reason sterility (Infertility) Ovulation disorder in women and spermatogenic problem in men. Factors that cause these problems include hypogonadism or dysfunction of the gonads, dysfunction of the hypothalamus-pituitary axis, inhibiting the function of the gonads, such as what is seen in hyperprolactinemia.

The cause of 25-30% of infertility is lack of ovulation, and the first line of treatment in these cases is clomiphene. 70% of patients respond to this. Because of the increased risk of ovarian cancer, more than 6 courses of baclomiphene are not recommended. Other treatment regimens include the use of clomiphene along with hCG and replacing clomiphene with tamoxifen. Tamoxifen is used in patients who cannot tolerate clomiphene or who have uterine mucus disorders. For patients who do not respond to clomiphene, the second line of treatment is the use of HMG.

In patients whose lack of ovulation is due to hypothalamus problems, the use of gnrho or its analogues in a pulse form causes fertility in 30% of cases.

An increase in the serum level of prolactin inhibits the secretion of gnrh and gonadal hormones and can cause infertility in any gender. The treatment in these patients is dopamine agonists such as bromocriptine and cabergoline.

The most common cause of infertility In men, spermatogenesis is disordered, and these patients are treated with diets containing hmg and hcg.

Other causes of infertility include:

1- Obstruction or inflammation in the fallopian tubes of women or the ducts of men (anti-inflammatory treatment such as corticosteroids)

2- The presence of antibodies against sperm

3- Endometriosis

4- Infections
